What Does a Case Manager Do? As a Case Manager with Lower Keys Medical Center, you will spend your day…

  • Coordinating the overall interdisciplinary plan of care for a patient, from admission to discharge.
  • Acting as a liaison between patient/family and healthcare personnel to ensure necessary care to ensure effective transition of care.
  • Attend Interdisciplinary Team (IDT) meetings of professionals who plan, coordinate and deliver personalized health care. 
  • Complete utilization reviews in a timely manner 
  • Assure completeness of cases for insurance purposes

  • Bachelor-level Social Worker or Mental Health Counselor
